Athletic Training Mission

ATTENTION ATHLETES

Fall sports begin soon.  Please remember to submit physicals to the athletic trainer's office prior to starting with any athletic team. The form can be downloaded from the school website under athletics or you can access it below.

This physical packet is for Returning Student Athletes Only


Athletic Physical Assessment 
The form below is included in the Athletic Physical Assessment, but not in the Freshman/Transfer Physical Packet.  If you child is a freshman or transfer, they will need the form below if playing a sport that requires them to wear a helmet.
